To open an account with CCII, please download a New CCII Account Kit. Complete, sign, and return the form to the address printed on the form.

Existing CCII Account Holders

If you have an existing CCII account(s) and wish to access your account(s) online, please download the Internet Agreement & Activation Form. Complete, sign, and return the form to the address printed on the form.

The following documents may be required when opening a CCII Non-standard Account or when adding features to your CCII existing account. Adobe Acrobat Reader is required to download and print these forms. Please call CCII Customer Service at 305-576-2200 if you have any questions.
